Enrollees

Welcome to the CT Behavioral Health Home website! A Behavioral Health Home is not a house or housing program. It is a place where you can receive services that will help improve your physical and mental health. These services will be provided to you by your current treatment and recovery team and with the help of others. These services are voluntary. Services are only available to people who want them. You will not be charged for these services. They are available to you because you are eligible for Medicaid.

  • “The Behavioral Health Home initiative has helped with coordinating my care between my clinician, physician, and nurse care manager. I have found new supports that have allowed me to maintain my sobriety. Also, I have found housing in a sober house and even obtained the position of House Leader. I am the happiest I have ever been.”

  • “Now I can get the care I need and focus on the most important factors: my health and recovery.”

  • “My Behavioral Health Home has helped me a lot. They helped me update my information with DSS and introduced me to a local social club. It is so good to get out every day. I got to be in the smoking cessation group and now I am not smoking at all. I never would have done that on my own. And the meditation group is wonderful. Plus I also am setting a lot better boundaries in my life.”
